Each ballot may contain a vote for only one finalist. Complete ballots should be mailed to: FAN Hall of Fame, Green Bay Packers, P.O. Box 10628, Green Bay, WI 54307-0628. Only one ballot should be included in each envelope, and the name of the finalist being voted for should be printed on the back side of the envelope.

Fans also can vote online at packers.com or jsonline.com.

All ballots must be received in the Packers office by Wednesday, December 13. The identity of this year's ultimate honoree will be announced on Friday, December 15.

The winner will receive four club seats to a Packers home game, at which he or she will be recognized on the Lambeau Field JumboTron; a $500 Packers Pro Shop gift certificate and a road trip for two to a 2001 Packers away game, including game tickets, air fare and hotel accommodation.

The honoree also will be introduced during the Packer Hall of Fame Induction Dinner next July and have his or her name permanently displayed in a place of honor in the Packer Hall of Fame.

Last year's FAN Hall of Fame honoree was 90-year-old Louis Gardipee of Black River Falls, Wis. The late Mel Knoke of Appleton, Wis., was the Hall's charter selection in 1998.
